A step-by-step care request form for a senior home-care service

I built a seven-step form where families request care. Its code has not changed since April 2024 and runs on the live home page today.

A regional German service helps older people at home. The service needed a guided form: the right questions one at a time, then an enquiry the manager can act on. The Elementor site was built by someone else, and it also broke on narrow phones.

I built a seven-step form on Contact Form 7 with custom JavaScript: service choice, a back button, a progress marker, small info pop-ups and a contact step. The place step has a map and suggestions by postcode or town that narrow as the visitor types. If a place is missing from the list, the form flags it and still lets the visitor go on. Two HTML emails go out: a confirmation for the family with a tap-to-call number, and one for the manager with every answer and the contact details. I built it all on an unpublished page and tested it with real emails before going near the live site.

For narrow phones I fixed clipped buttons and overflowing blocks and hid a reCAPTCHA badge that covered content, then tested it in Safari and Chrome. I also fixed a console error in a theme script. The request form runs on the live home page on the same code I delivered in April 2024, and the phone fixes from November 2024 are live too.

A read-more button the client sets with one tag

I wrote a small script that turns WordPress's standard more tag into a read-more button inside page-builder layouts, so the client sets it herself.
